Veterans’ Place Job Description for:
Housing Case Manager
Position: Full Time: Salary
Job duties include but are not limited to:
- Enter all residents in HMIS and Coordinated Entry System when they move into TVPI
- Meet with all residents within the first week of admission to make housing plans and fill out their Individual Housing Plan, to be reviewed and updated every quarter.
- Meet with individual residents to assist with all appropriate housing applications within the first month of moving into TVPI. Reply to denial letters to reverse decisions when needed.
- Keep all weekly progress notes and documentation on housing in Residents binders
- Meet with and coordinate with Case Managers when needed to discuss housing issues that come up. Barriers and potential issues that the CM’s can work on with the resident during case management meetings.
- Attend monthly Washington County Continuum of Care (COC), Coordinated Entry (CE), and Housing Retention (HRT) meetings in person or by video when appropriate. Attend Bi-weekly State Veterans Committee Case Conference calls, and any other meetings that pertain to housing issues.
- Keep HMIS CE and FBGT data bases updated and attend trainings for these data bases as needed.
- Meet with Housing Authorities and Landlords to advocate for residents.
- When needed take residents to look at apartments they are interested in.
- Attend morning house meetings
- Prepare Monthly reports for the Board
Qualifications:
- Must have 2-5 years’ experience with a housing back ground and working with Veterans and or homelessness, be understanding and compassionate with a multitude of mental health and or substance abuse issues
- Confidentiality for residents is a must
- Strong work ethic and a desire to succeed
- Must be flexible and able to work Monday through Friday
- Clean background check and reliable transportation
- Good communication skills and be able to multi task
Compensation/Benefits:
- Annual salary is $40,000.00 per year, paid bi-weekly;
- Medical insurance is provided on a cost share bases, 60% provided by TVPI; 40% provided by employee; the medical insurance plan is for one employee only; this option may be waived, if employee is qualified for Veteran Administration medical coverage, or other commercial medical insurance coverage;
- Eligible vacation is based on years of service using the following formula:
First Year - one week per year;
Second Year – one additional week;
Third year and thereafter vacation time accrues at 4 hours per pay period for a maximum of 30 days per year;
- Medical leave is unrestricted, but must be validated by a medical doctor.
- Sick leave accrues at 2 hours per pay period, per year. When accrued sick leave reaches 30 days, more time will not be accrued.
- Family Medical Leave is authorized but must meet the required guidelines and authorization.
Reporting:
The Housing Case Manager reports to the Managing Director.
